- Application for Retirement Benefits – to be completed no more than 90 days and not less than 30 days preceding retirement.
- Pension Estimate Worksheet – can be used to estimate your retirement benefit.
- Retirement Pension Description Form – provides an explanation of the various benefit options available.
- Direct Deposit Authorization Form – to be used to provide financial information for direct deposit of your retirement benefit. Please contact the Sheriffs’ Retirement System to obtain this form.
- Withholding Certificates for Pension or Annuity Statements:
- IRS Form W-4P – to be completed for Federal Withholding Amounts.
- Missouri Form MO W-4P – to be completed for State of Missouri Withholding Amounts.
- Address or Other Information Change Form – to notify the Sheriffs’ Retirement System of changes to the information on file for the member.
- Change of Beneficiary Form – for use by a retiree who selected a Life Income with Guaranteed Payments benefit option at retirement and wishes to change the designated beneficiary.
